'Service Unavailable' messages

This morning we had several calls about a 'Service Unavailable' message displaying on web sites. We looked into the issue and it was coming from a third party's server extension that was misbehaving. We have temporarily disabled the extension until we hear back from the vendor about why it caused the error, how to fix it, and how to make it not fail again.
